Etidronate, a synthetic analog of pyrophosphate, was the first bisphosphonate available in the US. Etidronate has been used in the treatment of Paget's disease, heterotopic ossification, malignant hypercalcemia, and osteoporosis. As a first-generation bisphosphonate, etidronate is associated with a lower potency and lower selectivity than second and third-generation bisphosphonates.
